Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Oklahoma City, OK. You will plan, direct, and provide OT services to residents under supervision, ensuring high quality care.

Full-time, part-time, or PRN options available. You will collaborate with an interdisciplinary team, follow the plan of care, document accurately, and maintain active OTA licensure through ongoing professional development.

Qualifications

• Associate's degree in Occupational Therapy.
• Must have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screenings of residents to determine best interventions.
• Perform treatment plans and follow short- and long-term goals.
• Interpret and communicate evaluation findings and recommendations to residents and families.
• Establish individual resident therapy schedules with team members.
• Conduct therapy according to plan and ensure proper documentation and billing of OT services.
• Comply with CMS documentation standards daily.
