Job summary

Pathways Healthcare is seeking a dedicated home health nurse to provide comprehensive patient care in Worcester and surrounding areas. You will perform assessments, administer treatments, and collaborate with physicians to develop Plans of Care, while ensuring high standards of patient safety and comfort.

The role emphasizes geriatrics, strong communication, and the ability to manage discharge planning and caregiver education. A valid state nursing license and OMS certification are required.

Qualifications

• Active nursing license in the state where the position is based.
• OMS (Ostomy Management Specialist) certification.
• 2+ years nursing experience with geriatrics preferred.
• Nursing degree from an accredited institution.
• Excellent bedside manners and communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families for home care needs.
• Perform thorough physical assessments and gather medical histories.
• Develop and revise individualized care plans based on patient status.
• Collaborate with physicians, NPs and teams to create Plans of Care.
• Address inquiries from patients, caregivers, and medical teams.
• Provide counseling and support before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and integrate them into care plans.

• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.

Qualifications:

• Must hold an active Nursing license in the State where the position is based.
• Must hold an active Ostomy Management Specialist (OMS) certification.
• Preferably 2+ years of nursing experience, with a focus on geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.
