Pathways Healthcare in Worcester, MA is seeking a per diem home health nurse to provide direct in-home care across Worcester and surrounding areas. The role includes initiating preventive and rehabilitative nursing procedures, administering prescribed medications, and performing initial patient assessments.

The ideal candidate holds an active nursing license in Massachusetts, OMS certification, and 2+ years of geriatric experience with strong communication and regulatory knowledge.

Qualifications

• Must hold an active Nursing license in the state where the position is based.
• Must hold an active Ostomy Management Specialist (OMS) certification.
• Preferably 2+ years of nursing experience with a geriatrics focus.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver’s license and own a reliable vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather medical histories.
• Develop and revise plans of care based on patient status and needs.
• Collaborate with physicians, providers, and teams to create robust plans of care.
• Address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers during treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and implement them as part of the care plan.
