Qualifications

• Bachelor’s Degree in Physical Therapy required.
• Licensed Physical Therapist (IL) with current credentials.
• BLS certification and ongoing CEU requirements.

Responsibilities

• Designs and modifies orthotic plans of care based on patient needs.
• Evaluates patients and documents outcomes and discharge plans.
• Educates patients and families using effective methods.
• Performs examinations and determines PT diagnoses and progress.
• Documents care per policy and uses EMR for daily notes.
• Collaborates with staff and students as a clinical resource.
