Hackensack Meridian Health's Jersey Shore University Medical Center seeks a Clinical Advocate to coordinate patient care, assess educational, financial and psychosocial needs, and connect families to resources. You will screen for clinical trial eligibility and collaborate with care teams throughout the patient journey.

Responsibilities include building referral networks, coordinating inpatient/outpatient care, educating patients and staff, and supporting performance-improvement initiatives

Formación

• New Jersey Nursing license is required.
• BSN is required; MSN preferred.
• Two years of nursing experience required.
• Orthopedics nursing experience preferred.
• National certification preferred.

Responsabilidades

• Coordinates care for a defined patient population, including educational, financial, psychosocial, nutritional and clinical needs.
• Connects patients and families to resources within the health system and community.
• Facilitates access to clinical trials and ensures eligibility is assessed.
