Job summary

Carle Health is seeking an RN Navigator to coordinate comprehensive surgical patient care across multiple specialties. You will work with interdisciplinary teams to ensure continuity of care, from diagnosis through follow-up, while supporting program development and quality improvement.

The ideal candidate is an RN with a BSN, BLS, and at least 2 years in Orthopedics, proficient in EMR, and capable of autonomous yet collaborative practice within a patient-centered care model.

Qualifications

• RN licensed with a BSN and BLS certification.
• Minimum 2 years of orthopedic RN experience.
• Proficiency with EMR and data management systems.

Responsibilities

• Coordinate and facilitate surgical patient care across specialties.
• Act as liaison among physicians, nursing staff, and patients/families to ensure seamless care.
• Navigate patients through diagnosis, treatment, and follow-up to support positive outcomes.
• Arrange referrals, scheduling, and follow-up appointments to optimize longitudinal care.
• Provide direct patient care as needed using the nursing process (assessment, planning, implementation, evaluation).
• Develop and update individualized, patient-centered care plans and educational materials.
