Job summary

Qualifications

Responsibilities

• Serve as a liaison between physicians, nursing staff, ancillary staff and patients/families to coordinate care.
• Navigate patients through diagnosis, surgical treatment and follow-up for positive outcomes.
• Facilitate referrals, scheduling and follow-up appointments to optimize care continuity.
• Provide direct patient care using the nursing process when needed.
• Develop, update and document individualized care plans.
• Educate patients and families about diagnoses, treatments, options and resources.
• Collaborate with leadership to implement clinical practice guidelines and care protocols.
• Monitor program performance metrics and patient outcomes.
