Qualifications

Responsibilities

• Coordinate care across specialties to optimize outcomes.
• Guide patients through diagnosis, surgical treatment and follow-up.
• Facilitate referrals, scheduling and follow-ups.
• Apply the nursing process to provide direct patient care as needed.
• Develop and maintain patient-centered care plans.
