Job summary

Carle Health is seeking an RN Navigator to coordinate comprehensive surgical patient care across specialties, acting as a resource to guide patients through diagnosis, treatment and follow-up. The role emphasizes collaboration with an interdisciplinary team to ensure high-quality, patient-centered experiences.

The RN will support program development, quality improvement and education initiatives, while ensuring effective communication with patients, families, and healthcare providers.

Qualifications

• RN with BSN and active RN license
• BLS certification within 30 days
• 2+ years of OR/Orthopedics RN experience
• Proficiency with EMR and data management systems

Responsibilities

• Liaise between physicians, nursing staff, ancillary staff and patients/families to improve care continuity.
• Navigate patients through diagnosis, surgical treatment and follow-up for positive outcomes.
• Arrange referrals, scheduling and follow-up appointments to support longitudinal care.
• Provide direct patient care using the nursing process as needed.
• Develop, update and document individualized patient-centered care plans.
• Deliver holistic care including education and psychosocial support.
• Educate patients and families about diagnoses, treatments and resources.
• Create and maintain educational materials tailored to patient needs.
• Promote patient engagement through evidence-based practices.
• Collaborate with leadership to implement clinical guidelines and care protocols.
• Monitor program performance metrics (quality, patient outcomes, surveys).
• Contribute to community outreach and multidisciplinary clinics.
• Maintain clear communication with patients and teams.
• Support staff development and competency training.
• Adhere to policies and regulatory standards, including point-of-care testing and quality control.
