Job summary

Carle Health is seeking an RN Navigator to coordinate comprehensive surgical patient care across specialties. You will bridge physicians, nursing staff and patients, guiding diagnosis, treatment and follow-up while supporting program development and education initiatives.

Responsibilities include developing individualized care plans, educating patients, and ensuring seamless care transitions with extensive EMR use and quality metrics monitoring.

Qualifications

• BLS certification within 30 days (AHA).
• Licensed Registered Nurse (RN) – Illinois licensure.
• BSN degree and 2+ years in Orthopedics as an RN.
• Strong communication, organizational and critical-thinking skills.
• Proficiency in conflict resolution, delegation, and time management.
• Comfort with electronic medical records (EMR) and data management systems.
• Ability to work autonomously while collaborating within a team.

Responsibilities

• Serve as liaison between physicians, nursing staff, ancillary staff and patients/families to enhance continuity of care.
• Navigate patients through diagnosis, surgical treatment and follow-up for positive outcomes.
• Arrange referrals, scheduling and follow-up to optimize longitudinal care.
• Apply nursing process to provide direct patient care as needed.
• Develop, update and document individualized care plans.
• Deliver holistic care including education and psychosocial support.
• Educate patients and families about diagnoses, treatment options and resources.
• Create educational materials tailored to patient needs.
• Promote patient empowerment through evidence-based practices.
• Collaborate with leaders and providers to develop care guidelines and protocols.
• Monitor program metrics, quality benchmarks and patient outcomes.
• Participate in community outreach and multidisciplinary clinics.
• Maintain clear communication with patients, families and the healthcare team.
• Serve as resource and mentor for staff education and competency development.
• Adhere to policies, procedures and regulatory standards including point-of-care testing.
