Job summary

HonorHealth is seeking a dedicated Physical Therapist at the Anthem location to evaluate and treat patients with physical disabilities and functional limitations. You will develop and update care plans, work with physicians, families, and staff to restore function, and participate in interdisciplinary rounds.

The role emphasizes evidence-based treatments, accurate documentation, and effective communication with caregivers and the care team to achieve optimal outcomes.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited university (APTA approved).
• Master's or Doctorate in Physical Therapy from an accredited university (APTA approved) preferred.
• BLS Certification required.
• AZ State Board of Physical Therapy license in good standing or PT Compact Licensure upon hire.

Responsibilities

• Implements individualized treatment plans using evidence-based PT techniques.
• Provides verbal and written instructions to patients and families.
• Maintains accurate documentation in medical records and billing sheets.
• Collaborates with interdisciplinary team and participates in rounds and case discussions.
