Job summary

HonorHealth in Anthem, AZ is seeking a dedicated Physical Therapist to evaluate and treat patients with functional limitations. You will develop care plans, collaborate with physicians, and contribute to quality patient outcomes.

The position requires a Bachelor's in Physical Therapy, a current AZ PT license, and BLS. A Master's/Doctorate and APTA credential are preferred. This is a day, on-site role with a sign-on bonus up to $15,000.

Qualifications

• Bachelors in Physical Therapy from an accredited program.
• Masters or Doctorate in Physical Therapy preferred.
• AZ State Board PT license in good standing required.

Responsibilities

• Evaluate, administer and oversee evidence-based physical therapy services.
• Develop and update individualized care plans with patient/family.
• Maintain documentation and ensure regulatory compliance.
