Job summary

Premise Health Inc. seeks a Full Time Collaborative Practice Pharmacist for our Health Center in Orlando, FL. You will provide direct patient care, implement medication management, and support collaborative care agreements to optimize patient outcomes.

Clinic hours are Mon–Fri, 7a–7p, with a start date in late April 2027. You will lead education, preventive care, and wellness initiatives, while collaborating with providers and staff to improve productivity and patient satisfaction.

Qualifications

• Pharm D or pharmacist degree with active state license.
• Experience in disease state management or clinical residency.
• Strong customer service and education program development.
• Experience coaching staff and health promotion activities.
• Familiarity with EMR and data reporting.

Responsibilities

• Provide direct patient care to improve outcomes.
• Medication management and reconciliation.
• Education and behavioral counseling.
• Preventive care screening per state regs.
• Collaborative care agreements under protocol.
• Serve as pharmacology expert for staff.
• May perform additional duties as assigned.
