Job summary

Premise Health seeks a Full Time Collaborative Practice Pharmacist to provide direct patient care for employees and dependents at our Health Center in Orlando, FL. Clinic hours are Monday–Friday, 7am–7pm, with a start date in late April 2027.

The role focuses on medication management, reconciliation, education, preventive care, and collaborative practice to improve outcomes and provider productivity within a defined protocol.

Qualifications

• Pharm D or Bachelor of Pharmacy Degree.
• Current and unrestricted pharmacist licensure.
• Certified Diabetic Educator or willingness to obtain within one year.
• Coaching certificates or equivalent advanced training in motivation and leading change, or willingness to obtain.

Responsibilities

• Medication Management to collect histories, perform reviews, monitor therapy and relay recommendations.
• Medication Reconciliation to prevent errors during therapy changes and observe adherence.
• Education and Behavioral Counseling for patient education, one-on-one or group sessions.
• Preventive Care Services including cholesterol and HbA1c measurements and related labs.
• Collaborative Care Practice Agreements allowing protocol-based patient assessments and drug therapy management.
• Serve as resident pharmacologic expert for other staff providers.
• May require other duties as assigned.
