Responsibilities

• Performs initial and ongoing assessments of patient condition.
• Establishes a care plan in collaboration with the patient and family.
• Monitors, records, and communicates patient status to the care team and physicians.
• Coordinates patient care across disciplines, including transfers and discharges.
