Job summary

On Your Mark seeks a CAS Coordinator to coordinate the Coordinated Assessment System for individuals served by OPWDD. You will liaise with assessors, care managers, clients and families to ensure timely assessments and accurate CAS data for service planning.

The role requires a Master’s degree in a relevant field and at least five years of experience with ID/DD populations, OPWDD regulations, and CAS. Strong computer skills and staff training ability are essential.

Position Overview:The OPWDD Coordinated Assessment System (CAS) Coordinator is responsible for coordinating, monitoring, and supporting all activities related to the Coordinated Assessment System for individuals receiving services through the New York State Office for People with Developmental Disabilities (OPWDD). The Coordinator serves as the primary liaison between OPWDD assessors, program staff, Care Managers, individuals receiving services, and family members to ensure timely completion of assessments and accurate utilization of CAS information in service planning.Position Responsibilities include:Essential Duties and Responsibilities:Coordinate and track all scheduled CAS assessments, reassessments, and annual reviews.Serve as the primary point of contact for OPWDD CAS assessors.Ensure individuals, families, guardians, and staff are informed of upcoming assessments.Facilitate assessment appointments and provide necessary documentation to assessors.Maintain a tracking system to monitor assessment of due dates and completion status.Documentation and Compliance:Review CAS reports for accuracy and completeness.Ensure CAS results are incorporated into Life Plans, Staff Action Plans, and other service planning documents.Maintain organized and confidential records in accordance with HIPAA, OPWDD regulations, and agency policies.Monitor compliance with OPWDD requirements related to assessments and service planning.Ensure compliance with all On Your Mark, Inc., OPWDD and New York State Justice Center policies and regulations.Intervene appropriately and report any incidents or suspected incidents of abuse and/or neglect IMMEDIATELY to a supervisor.Assume additional responsibilities as assigned by the Director of Clinical Services and the Executive Team.Collaboration and Communication:Collaborate with Care Managers, clinicians, program managers, residential staff, and day-habilitation staff regarding assessment outcomes.Communicate assessment findings and recommendations to appropriate stakeholders.Attend interdisciplinary team meetings and support planning discussions based on CAS results.Assist in resolving assessment-related concerns or discrepancies.Interact professionally and courteously with individuals, parents, co-workers, other professionals, and management.Training and Support:Provide formalized ongoing training and guidance to agency staff and families as needed regarding CAS procedures and requirements.Educate staff on the interpretation and practical application of CAS outcomes.Support quality improvement initiatives related to assessment and person-centered planning processes.Education:Master’s Degree in human services, Psychology, Social Work, Rehabilitation, Special Education, Healthcare Administration, or a related field required.Experience:Minimum of 5 years of experience working with individuals with intellectual and developmental disabilities.Experience with OPWDD regulations, services, and person-centered planning.Knowledge of the Coordinated Assessment System (CAS) and OPWDD service delivery systems.Experience with quality assurance, case management, or program coordination.Good computer skills including the ability to learn new software applications and teach staff new software applications.Reports to:Director of Clinical ServicesExecutive Team
