On Your Mark in Staten Island, NY seeks a part-time OPWDD Coordinated Assessment System (CAS) Coordinator. Qualifications

  • Master’s Degree in human services, Psychology, Social Work, Rehabilitation, Special Education, Healthcare Administration, or a related field required.
  • Minimum of 5 years of experience working with individuals with intellectual and developmental disabilities.
  • Experience with OPWDD regulations, services, and person-centered planning.
  • Knowledge of the Coordinated Assessment System (CAS) and OPWDD service delivery systems.
  • Experience with quality assurance, case management, or program coordination.

Responsibilities

  • Coordinate and track all scheduled CAS assessments, reassessments, and annual reviews.
  • Serve as the primary point of contact for OPWDD CAS assessors.
  • Ensure individuals, families, guardians, and staff are informed of upcoming assessments.
  • Facilitate assessment appointments and provide necessary documentation to assessors.
  • Maintain a tracking system to monitor assessment due dates and completion status.
  • Review CAS reports for accuracy and completeness and ensure CAS results are incorporated into Life Plans and related documents.
  • Maintain organized and confidential records in accordance with HIPAA, OPWDD regulations, and agency policies.
  • Monitor compliance with OPWDD requirements related to assessments and service planning.
  • Interact professionally with individuals, families, co-workers, and management.
