Job summary

On Your Mark seeks a CAS Coordinator to coordinate the Coordinated Assessment System for individuals served by OPWDD. You will liaise with assessors, care managers, clients and families to ensure timely assessments and accurate CAS data for service planning.

The role requires a Master’s degree in a relevant field and at least five years of experience with ID/DD populations, OPWDD regulations, and CAS. Strong computer skills and staff training ability are essential.

Qualifications

• Master’s degree in a relevant field is required.
• Minimum of 5 years of experience working with individuals with intellectual and developmental disabilities.
• Experience with OPWDD regulations, CAS, and person-centered planning.
• Strong computer skills and ability to learn new software and teach staff.

Responsibilities

• Coordinate and track all CAS assessments, reassessments, and annual reviews.
• Serve as the primary liaison between OPWDD assessors, care managers, individuals and families.
• Ensure CAS results are incorporated into Life Plans and service planning documents.
• Maintain organized, confidential records in accordance with HIPAA and OPWDD policies.
• Monitor compliance with OPWDD requirements and report incidents as needed.
