Oaks Integrated Care Inc. in Burlington County, NJ seeks a Patient Navigator to lead the Opioid Overdose Recovery Program (OORP).

You will supervise staff, coordinate care, and connect survivors with treatment and community resources, ensuring a comprehensive recovery plan and collaboration with EMO providers. The role requires a Bachelor’s degree in human services, 3+ years in behavioral health, supervisory experience preferred, and a valid driver’s license.

Qualificações

• Bachelor's degree in human services or related field.
• Master's degree preferred.
• At least 3 years of experience in behavioral health.
• Supervisory experience preferred.
• Valid Driver's License required.

Responsabilidades

• Oversee the Opioid Overdose Recovery Program (OORP) and supervise Peer Recovery Support staff.
• Conduct weekly group supervision and regular 1:1 meetings with staff.
• Collaborate with the team to support survivors' recovery goals.
• Navigate services with EMO and external providers to meet survivor needs.
• Support survivors in choosing and accessing treatment options.
• Develop and monitor individual recovery plans and referrals.
• Train ED staff on program protocols and procedures.
• Document all interactions and maintain EHR accuracy.
• Promote recovery-oriented services and family engagement.
• Provide 24/7 on-call support during program hours and high-volume periods.
• Lead program evaluation and report outcomes to the evaluator.
• Recruit, train, and develop the program team.
