Responsibilities

• Assess physical, social, and psychological status and gather information from the care team.
• Plan and develop individualized care plans with patient and family involvement.
• Coordinate care through delegation and collaboration with the interdisciplinary team.
• Evaluate patient response and revise care plans as needed.
• Promote professionalism, ethical practice, and accurate documentation.
