Job summary

Responsibilities include assessing physiologic and psychosocial health, developing care plans, administering medications, and educating patients and families.

Qualifications

• Graduate from an accredited School of Nursing.
• Associate’s Degree in Nursing required; Bachelor’s preferred.
• Current NYS license to practice as a Registered Professional Nurse.

Responsibilities

• Assess the patient’s physiologic health status and pain level.
• Assess psychosocial health status and coping needs.
• Identify patient outcomes and develop individualized goals.
• Formulate and prioritize the plan of care; coordinate supplies and meds.
• Implement the plan of care; monitor safety and revise as needed.
• Evaluate care outcomes and document results for quality improvement.
• Collaborate with physicians and care team; delegate tasks appropriately.
• Perform skin care (grade I-IV decubitus) and wound care as ordered.
• Administer medications accurately and educate about interactions.
• Educate patient and family; support discharge planning and referrals.
• Perform related duties as required.
