Brooksby Village by Erickson Senior Living is seeking a Per Diem on-site Out-patient Physical Therapist to support our residents in a continuum of care within a vibrant campus setting.

You will implement personal care plans, focus on residents’ strengths, and collaborate with an interdisciplinary team to address physical, emotional, and clinical needs. Onsite physicians support resident care, with a comprehensive benefits package offered.

Formación

• Must possess a valid and current state Physical Therapy license.
• Ability to complete documentation for reimbursement and regulatory requirements.
• Experience with older adults across multiple diagnostic groups preferred.

Responsabilidades

• Implement residents’ personal care plans focusing on strengths and preferences.
• Support residents’ physical, spiritual, emotional, and clinical needs.
• Collaborate with interdisciplinary clinical team to address resident needs.
• Complete documentation for reimbursement and regulatory requirements.
