Job summary

Brooksby Village by Erickson Senior Living seeks a Per Diem on-site Out-patient Physical Therapist to join our campus team. Residents live on-site and receive comprehensive care aligned with their personal plans, routines, and comfort.

You will collaborate with an interdisciplinary team to support physical, emotional, and clinical needs, fostering a sense of belonging and wellbeing. The role emphasizes performance with documentation, regulatory compliance, and experience in older adults.

Qualifications

• Must possess a valid and current state Physical Therapy license.
• Ability to complete documentation to meet reimbursement and regulatory requirements.
• Therapeutic experience in older adults across multiple diagnostic groups is preferred.

Responsibilities

• Provide holistic Physical Therapy to residents.
• Implement residents’ personal care plans focusing on individual strengths and routines.
• Collaborate with the interdisciplinary clinical team to address residents’ needs throughout their care.
• Support wellness and preventive strategies within the on-site campus setting.
