Job summary

Primary Care Solutions is seeking a Registered Nurse to join our Veterans-focused team in Ponca City, Oklahoma. As part of a Patient Aligned Care Team (PACT), you will contribute to developing and implementing patient care plans and deliver high-quality clinical care onsite.

Office hours are Monday through Friday, 8:00 am to 4:30 pm, with full-time on-site responsibilities. You will assist practitioners, triage same-day needs, coordinate care, educate patients and families, and help meet

Qualifications

• 1 year of Primary Care and/or Chronic Disease Management experience
• Active, unrestricted state Nursing License
• BLS for providers certification (American Heart Association)
• Strong computer skills, including EMR experience

Responsibilities

• Assist practitioners with patient care and nursing tasks.
• Triage urgent/same-day patients and identify acuity.
• Coordinate patient navigation and care planning.
• Participate in daily team huddle and communication.
• Answer patient calls, messages and alerts.
• Provide patient/family education on self-management and wellness.
• Follow up on discharge lists and patient contacts.
• Maintain welcoming, professional atmosphere for patients and colleagues.
• Support achievement of clinical goals and metrics.
