Qualifications

• Active and unrestricted RN license in Texas.
• 3+ years of clinical experience, preferably with Medicare and commercial populations.
• Ability to work independently within a hospital-based environment.
• Strong analytical, problem-solving, organizational, and communication skills.
• Proficiency with MS Word, Excel, Outlook, PowerPoint and clinical systems.
• Reliable transportation and onsite presence at Houston Methodist West Hospital (50–75%).

Responsibilities

• Serve as liaison between members, families, providers, and insurers.
• Coordinate case management for catastrophic and chronic conditions across care continuum.
• Interact with members onsite and via telecommunication.
• Assess medical, functional, and vocational status to plan individualized care.
• Document all case management activities timely and accurately.
• Collaborate with multidisciplinary teams to support outcomes.
• Engage with treating physicians to support care planning.
• Provide education and preventive guidance to promote health outcomes.
