Job summary

CVS Health Corporation is seeking an on-site Case Manager to join the ACC team at Houston Methodist Hospital in Texas. The role focuses on assessment, care planning, coordination, and advocacy to address comprehensive health needs of individuals and families.

Requirements include an active Texas RN license and 3+ years of clinical experience. You will work 40 hours per week in a hospital setting, coordinating with care teams and ensuring timely, high-quality outcomes.

Qualifications

• Active and unrestricted Texas RN license.
• 3+ years of clinical experience.
• Ability to work independently within a hospital-based environment.
• Strong analytical, problem-solving, organizational, and communication skills.
• Proficiency in MS Word, Excel, Outlook, PowerPoint, and proprietary clinical systems.
• Reliable transportation and ability to meet onsite hospital-based work requirements at Houston Methodist Hospital up to 50-75%.

Responsibilities

• Serve as liaison between members, families, providers, and healthcare personnel.
• Coordinate case management activities for catastrophic and chronically ill members.
• Interact with members in person at the hospital and via telecommunication.
• Assess medical, functional, and vocational status to develop individualized plans of care.
• Communicate with stakeholders including medical providers, attorneys, and insurers.
• Document all case management activities accurately and timely.
• Collaborate with multidisciplinary teams to support outcomes.
• Provide education and preventive guidance to promote outcomes.
