Job summary

Barnes-Jewish Hospital in St. Louis, MO is seeking an RN with case management responsibilities for an onsite, PRN role. The position involves 8-hour shifts, 7am or 8am start times, and a schedule of two shifts every four weeks.

No weekends or holidays, with shifts arranged to support inpatient discharge planning. The CM Services unit collaborates with physicians and interdisciplinary teams to ensure safe discharge and optimal patient care.

Qualifications

• RN license required or registered nurse licensure.
• BSN degree preferred.
• 2–5 years RN experience, ideally in inpatient/case management.

Responsibilities

• Assesses, coordinates, and evaluates each patient's plan of care.
• Establishes, continuously evaluates, and documents an individualized discharge plan.
• Ensures clinical documentation and cost-effective use of hospital resources.
