The role emphasizes collaborative work with physicians and interdisciplinary teams to achieve quality, cost-effective outcomes and safe discharge planning. Preference given to BSN holders with prior case management experience.

Qualifications

• At least 2 years of RN experience, ideally in an inpatient environment.
• BSN preferred.
• Prior case management experience.

Responsibilities

• Assesses, coordinates, and evaluates each patient's plan of care.
• Establishes, continuously evaluates, and documents an individualized discharge plan.
• Ensures clinical documentation and cost-effective use of hospital resources.
