Job summary

CompuGroup Medical, Inc. seeks an on-site Medical Assistant – Care Coordinator in Monroe, LA to support Medicare RPM and CCM patients. You will engage high-risk patients with structured outreach, monitor vitals, and ensure adherence to personalized care plans in a patient-centered setting.

The role emphasizes clear education on program benefits, thorough documentation in EHRs, and coordination of device scheduling, all within a stable Monday–Friday daytime schedule.

Qualifications

• Active NCMA credential required.
• Minimum 2 years healthcare experience.
• Minimum 1 year in Chronic Care Management services.
• Strong communication and interpersonal skills.
• Comfortable with high-volume outbound calling.
• Strong documentation and EHR proficiency.
• Willingness to work on-site Monday–Friday.

Responsibilities

• Make 60+ outbound calls per day to eligible patients using a dialer.
• Accurately document call outcomes in multiple EHR/systems.
• Conduct monthly RPM and CCM check-in calls.
• Review and monitor vitals from RPM devices.
• Explain RPM program benefits clearly to patients.
• Schedule and order RPM devices and educate patients on usage.
• Obtain verbal enrollment consent per program requirements.
• Track and document patient progress accurately.
