Job summary

CompuGroup Medical SE & Co. KGaA is hiring a Care Coordinator/Medical Assistant to support RPM and CCM patients. You will be the primary contact for high-risk patients, performing outbound outreach, documenting interactions in EHRs, and monitoring vital signs.

This on-site role operates Monday through Friday with no evenings or weekends. The ideal candidate has at least 2 years in healthcare, CCM exposure, and NCMA certification.

Qualifications

• Active National Certified Medical Assistant (NCMA) required.
• Minimum 2 years of healthcare experience.
• At least 1 year in Chronic Care Management
• Excellent communication and interpersonal skills.
• Comfortable with high-volume outbound calling.
• Strong documentation and EHR proficiency.
• Willingness to work on-site Monday to Friday.

Responsibilities

• Make 60+ outbound calls per day to eligible patients using our dialer system.
• Accurately document call outcomes in multiple client EHRs and systems.
• Conduct monthly check-in calls for RPM and CCM patients.
• Review and monitor vital signs synced from RPM devices.
• Explain RPM program benefits in patient-friendly language.
• Schedule and order RPM devices and educate patients on usage.
• Verify patient contact details and enrollment consent per program rules.
• Perform structured health assessments and tailor care plans.
• Provide education on nutrition, exercise, medications, and lifestyle.
• Document patient progress consistently and accurately.
