Qualifications

• Active NCMA credential required.

• Strong documentation and EHR proficiency.
• Willingness to work on-site Monday–Friday.

Responsibilities

• Make 60+ outbound calls per day to eligible patients using a dialer.
• Accurately document call outcomes in multiple EHR/systems.

We are seeking a motivated and patient-focused Care Coordinator to support patients enrolled in Medicare’s Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs. This is not a traditional back-office Medical Assistant role. Instead, you will serve as a key point of contact for high-risk patients with multiple chronic conditions — helping them stay engaged, compliant, and on track with their care plans through structured outreach and monitoring.
