Job summary

CompuGroup Medical, Inc. is seeking a Medical Assistant – Care Coordinator in Lebanon, VA for an on-site, Monday–Friday role with no evenings or weekends.

You will support Medicare RPM and CCM programs, acting as a primary contact for high-risk patients with chronic conditions to keep them engaged and compliant with care plans. You will perform structured outreach, monitor device vitals, educate patients on device use and medications, and document progress in multiple EHR systems.

Qualifications

• Active National Certified Medical Assistant required.
• Experience in healthcare with CCM/RPM programs is preferred.

Responsibilities

• Make 50+ outbound patient calls per day.

• Conduct monthly check-in calls for RPM and CCM patients.

Instead, you will serve as a key point of contact for high-risk patients with multiple chronic conditions — helping them stay engaged, compliant, and on track with their care plans through structured outreach and monitoring.
