Croí Health is seeking an experienced, compassionate, and detail‑oriented Licensed Practical Nurse (LPN) to join our team as a Triage & Wellness Nurse.

In this vital role, you will serve as a key point of contact for patients, families, physicians, and clinical staff by managing live triage calls, coordinating communication, documenting critical information, and supporting patient wellness through outreach and follow‑up calls.

Schedule: Full‑Time, 40 hours per week, onsite in Norwell

Compensation: $35.00 – $42.00 per hour based on experience

How You'll Make an Impact

• Answer and manage live triage and physician telephone lines, ensuring timely and accurate communication with clinical staff.
• Assess incoming calls and appropriately route information from physicians, patients, caregivers, and family members to the appropriate clinical manager or team member.
• Conduct wellness calls to patients for appointment reminders, follow‑up care, patient education, and general well‑being checks.
• Coordinate, document, and communicate laboratory results to clinicians, physicians, patients, and family members as directed.
• Serve as a communication liaison between patients, referral sources, physicians, and internal clinical teams.
• Maintain accurate and timely documentation within agency systems.
• Support quality assurance initiatives and maintain established service standards.
• Assist in reviewing, analyzing, and tracking clinical referral and patient data as needed.
• Ensure compliance with all agency policies, regulatory requirements, and quality standards.
• Perform additional duties and special projects as assigned.

What You'll Bring to the Team

• Current Massachusetts LPN License in good standing
• Graduate of an accredited Licensed Practical Nursing (LPN) program
• Minimum of five (5) years of recent nursing experience
• Minimum of three (3) years of community health or home health experience preferred
• Knowledge of Medicare and Managed Care regulations and processes
• Strong organizational and time‑management skills
• Excellent verbal and written communication skills
• Outstanding customer service and interpersonal skills
• Ability to effectively prioritize, problem‑solve, and multitask in a fast‑paced environment
• Strong analytical and troubleshooting abilities
• Proficiency with computers, electronic medical records, and keyboarding
• Ability to communicate professionally and compassionately with patients, families, physicians, and staff
