Job summary

The People Concern is seeking an ICMS Case Manager in Los Angeles to support clients facing homelessness through evidence-based practices. This full-time role is temporary until 11/30/2026, with possible transfer opportunities.

You will maintain a caseload, develop independence in clients, and collaborate with healthcare teams while working in both field and office environments. A high tolerance for a harm reduction model is essential.

Qualifications

• Minimum two years' experience working with homeless individuals, especially with mental illness or substance addictions.
• Valid CA Driver's license and ability to transport Clients.
• Able to work in a harm reduction model.

Responsibilities

• Maintain caseload of approximately 20 high-acuity clients for intensive services.
• Collaborate with multidisciplinary teams to enhance client treatment.
• Complete biopsychosocial assessments for individualized planning.
• Support Clients by teaching life skills for successful transitions.
