Job summary

Responsibilities

• Rounds on and review assigned patients, update the plan of care with the patient and family.
• Provide emotional support, clinical education, and guidance to patients and families.
• Coordinate between patient, clinical staff, administration, physicians and external resources to resolve barriers to the plan of care.
• Assist patients with resolving financial, psycho-social, and administrative issues and advocate as needed.
