Job summary

Humana is seeking an Onsite Care Coordination Nurse to support members during transitions from hospital to home. You will conduct face-to-face assessments, coordinate discharge planning, and collaborate with care teams in a hybrid role at Shannon Medical Center in San Angelo, TX, with some remote documentation.

Ideal candidates are Texas-licensed RNs with acute care experience, strong communication, and familiarity with EMR systems.

Qualifications

• Current, unrestricted RN license in Texas.
• Located within 30–60 minute driving distance to Shannon Medical Center.
• Minimum of 3 years acute care nursing experience.
• Experience with discharge planning or care coordination in hospital or health plan.
• Experience in MS Office and EMR.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Conduct comprehensive, face-to-face assessments of newly admitted members at assigned acute care facilities.
• Support and educate members and families on discharge planning from admission through post-discharge.
• Identify gaps in care and barriers to discharge; develop strategies to address these issues.
• Collaborate with attending providers, facility case managers, and other healthcare professionals to ensure timely, coordinated care and discharge planning.
• Provide education on diagnoses, medications, and preventive measures to promote wellness and reduce readmission.
• Complete all required documentation accurately and promptly.
• Maintain adherence to infection control, safety, and patient privacy standards.
