Qualifications

• Graduate of an accredited Physical Therapy program.
• Current or eligible Illinois Physical Therapist license.

Responsibilities

• Provide one-on-one outpatient PT evaluations and follow-ups.
• Ensure thorough documentation and adherence to the clinic schedule (60‑min evaluations, 30‑min follow-ups).
• Participate in Duly's mentorship program and pursue clinical specialty development.

Education
