Qualifications
- Must have a valid Arizona driver's license and reliable transportation.
- Current TB test or chest X-ray must be within the past 12 months.
- Willingness to take additional precautions to minimize exposure risks.
Responsibilities
- Provide skilled nursing care in the patient's home.
- Follow physician orders and individualized care plans.
- Administer medications and treatments as prescribed.
- Perform patient assessments and monitor changes in condition.
- Complete accurate and timely clinical documentation.
- Communicate effectively with physicians, family, and the clinical team.
- Assist with activities of daily living as appropriate.
- Maintain a safe, comfortable, and supportive environment.
