Job summary

Angels of Care Pediatric Home Health is seeking Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide one-on-one home health care for children in Mesa, AZ and surrounding areas.

Position involves pediatric home visits, care coordination, family education, and use of prescribed treatment plans with a focus on compassionate, patient-centered services. Training is provided for new grads, and a supportive team environment is emphasized.

Qualifications

• Active RN or LPN license (New Grads Welcome, training provided!)
• Provide care in a client home setting
• Ability to make a positive and lasting impression!

Responsibilities

• Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel.
• Conduct on-going patient care and assessments.
• Administration of prescribed medication, treatments, and therapies.
• Coordination of care
• Educate family members on patient clinical care to enhance positive outcomes
• Preventative initiatives to protect quality of care for patient
