Job summary

Inova Fairfax Cancer Center is seeking an Experienced Social Worker Case Manager to join the Case Management Team to support Pediatric patients. This full-time role is Monday–Friday, 8:00am–4:30pm, with no evenings or weekends.

The role involves assessing biopsychosocial needs, coordinating discharge planning, and advocating for patient/family access to services, while collaborating with the interdisciplinary care team in Northern Virginia.

Qualifications

• Master's Degree in Social Work required.
• Minimum 1 year of experience in clinical care or clinical case management.
• Basic Life Support (BLS) for Healthcare Provider certification from the American Heart Association required at start.

Responsibilities

• Participates in assessment of patients' biopsychosocial needs and communicates care plans.
• Ensures options to support transition and continuity of care are communicated and documented.
• Assesses psychosocial risk factors and intervenes regarding emotional, social and financial consequences.
• Provides discharge planning and coordinates referrals to clinics, home health, hospice, and post-acute services.
• Collaborates with the care team and families in discharge planning and post-discharge needs.
