Job summary

Jefferson Health Northeast seeks a social worker to assess and enhance patients’ and families’ coping with illness and discharge needs. You will work with a multidisciplinary team to establish effective discharge planning and provide psychosocial counseling throughout hospitalization.

Role requires MSW and hospital social work experience, with strong knowledge of medical terminology, resources, and insurance requirements.

Qualifications

• Master’s Degree in Social Work from an accredited program.
• 2 years hospital-based social work experience.
• Knowledge of medical terminology, disease processes, and insurance regulations.
• Ability to establish safe discharge plans and coordinate with diverse teams.

Responsibilities

• Interacts with co-workers, visitors, and other staff per Jefferson Health values.
• Conducts psychosocial assessment related to patients' social, emotional, and cognitive needs.
• Develops and implements intervention plans addressing identified needs and discharge planning.
• Provides counseling to patient/family to assist with coping with diagnoses and end-of-life issues.
• Advocates to obtain necessary services and coordinates team and family conferences.
• Completes referrals to community-based services and documents in the EMR.
