Jefferson Health Northeast is hiring an Oncology Social Worker in Northeast Philadelphia for a full-time role. The position focuses on psychosocial assessment, discharge planning, and coordination with the multidisciplinary team to support patients and families through illness, treatment, and post-acute care.

The specialist provides counseling, advocates for patient needs, and ensures appropriate services are secured across hospital and community settings.

Formación

• Master’s degree in social work from an accredited program.
• Two years hospital-based social work experience.
• Knowledge of medical terminology, community resources, and insurance regulations.
• Ability to coordinate discharge planning with the multidisciplinary team.

Responsabilidades

• Interacts with staff and visitors in alignment with Jefferson values.
• Performs psychosocial assessments relating to illness and discharge needs.
• Develops and implements intervention plans addressing identified needs.
• Provides counseling to patients and families to cope with diagnoses and palliative or end-of-life care.
• Advocates for patients to obtain necessary services and coordinates with community resources.
• Documents in the electronic medical record and coordinates with case managers.
• Facilitates weekend discharge planning and post-acute care arrangements.
