Job summary

Jefferson Health is seeking a Social Worker to provide comprehensive psychosocial services to patients and families. The role involves assessing coping abilities, coordinating discharge planning, and collaborating with a multidisciplinary healthcare team to ensure safe transitions of care.

Minimum qualifications include an MSW and at least two years of hospital-based social work experience. The position requires counseling, crisis intervention, and strong advocacy for patient needs within Center

Qualifications

• Master’s degree in social work required.
• 2 years hospital-based social work experience required.
• Knowledge of medical terminology and disease processes.
• Ability to establish discharge plans and coordinate with multidisciplinary teams.
• Experience providing counseling and crisis intervention.

Responsibilities

• Provides social work services for patient/family related to illness, discharge planning and coping.
• Conducts psychosocial assessments and reassessment of patients and families.
• Develops and implements interventions and coordinates with case managers and external resources.
• Provides psychosocial counseling, crisis intervention, and end-of-life support.
• Precepts MSW students and mentors new staff as needed.
• Facilitates family conferences and discharge planning with the care team.
