Jefferson Health Northeast is hiring an Inpatient Care Manager-MSW to coordinate comprehensive care across the continuum. The role involves assessing, planning, implementing, and evaluating individualized care plans with safe and timely discharge planning.

The position requires an MSW and strong collaboration with the interdisciplinary team to achieve high-quality outcomes, with knowledge of Medicare/Medicaid and payer reimbursement.

Qualifications

• MSW degree required with less than 1 year of related MSW internship experience.
• Ability to coordinate care plans and discharge planning within a multidisciplinary team.
• Knowledge of Medicare/Medicaid and payer reimbursement concepts.

Responsibilities

• Conduct comprehensive assessments and develop individualized plans of care.
• Coordinate safe and timely discharge planning including post-acute needs.
• Lead multidisciplinary rounds and address barriers to discharge.
• Advocate for patients and families and coordinate community resources.
• Maintain knowledge of payer systems and assist with access to services.
