Qualifikationen

Aufgaben

• Interact with co-workers, visitors, and other staff consistent with the values of Jefferson.
• Conduct psychosocial assessment related to social, emotional and cognitive needs of patients.
• Develop and implement intervention plans addressing identified needs with the multidisciplinary team.
• Advocate for patients and families to obtain necessary services.
• Provide counseling to patients/families to support coping with diagnoses, chronic illness, or end-of-life care.
• Coordinate discharge planning, including home care, hospice services, transportation, and post-acute facility placements.
• Document updates in the EMR per department procedures.
