Job summary

Mercyhealth is seeking a Social Worker to provide counseling, information, and directions to obtain community resources for oncology patients and their families. You will collaborate with physicians, nurses, and the allied health team to support care planning and resource access.

The role requires a BSW, 1 year healthcare social work experience preferred, and ability to facilitate groups. The position may involve flexible hours and is based in Illinois/Wisconsin Mercyhealth facilities.

Qualifications

• BSW from an approved graduate school of social work.
• One year social work experience in a healthcare setting preferred.
• Experience and ability to facilitate groups.

Responsibilities

• Assess patients for continuing care needs and create care plans in coordination with the physician and care team.
• Refer patients to community resources and assist with advance directives.
• Educate patients about resources and document all activities in a timely manner.
• Provide emotional support and help patients and families understand their condition and medical recommendations; arrange changes in levels of care and setting as needed.
• May work flexible hours.
