Job summary

Mercy Health is seeking an Advanced Social Worker in Physician Practices - Oncology to provide high-level support, advocacy and care planning for patients facing complex physical, mental and emotional challenges. This role involves clinical assessment, counseling, and collaboration with other professionals to ensure comprehensive care.

Advanced Social Workers may work across settings, with a focus on empowering patients and advocating for their well-being.

Qualifications

• Master's degree in Social Work (MSW) required.
• LSW license in state of practice required; LCSW or LISW preferred.
• Minimum 2 years in social or human services, including direct practice, assessment and treatment planning.
• Hard/tech/clinical skills include debriefing, motivational interviewing, and patient-centered care.

Responsibilities

• Evaluates patient's needs and builds individualized care plans.
• Develops advanced intervention strategies for complex patient populations.
• Provides emotional support, counseling and education to help patients cope.
• Manages cases, tracks progress and refers to community resources.
• Represents patients' interests and coordinates with a multidisciplinary team.
