Job summary

Qualifications

• Must possess a current RN license in state of practice; BLS certification is required.
• Experience in an acute care hospital setting is preferred.

Responsibilities

• Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs.
• Formulates and updates plan of care in collaboration with patient/family and health team.
• Implements the plan of care with accurate documentation and coordination of care.
• Evaluates patient and family response to care and teaching, revising plans as needed.
• Documents assessments, planning, implementation and evaluation in the patient record; maintain legibility and timeliness.
