• Consults with patients’ physicians about high-priority patients and care plans.
• Initiates contact to perform initial assessment and develop a care management plan.
• Coordinates care across home-based services and hospital resources.
• Documents interventions and progress in electronic health records.
• Collaborates with physicians, nurses, and care teams to optimize patient outcomes.
