Qualifications

Responsibilities

• Direct Patient Care: intervene to improve prescribing, administration, and monitoring.
• Medication Therapy Management: provide MTM services and patient education.
• Dosing recommendations based on patient factors and organ function.
• Oversee preparation of chemotherapy and collaborate with hematology/oncology teams.
• Participate in rounds, trials, and interdisciplinary care to support safe therapies.
